      That reminds me of the (American) pro football players who have their brains damaged after a long time of bashing their heads against helmets. They don't get normal type concussions but their brains eventually develop something called tau cells, which causes neurodegenerative disorders (dementia). Even though everyone knows this happens, they still persist and don't switch to something like rugby, because they make more money playing football 😉 and football players are the true warriors!!! This orc warrior is actually a bit realistic because even humans have weird exaggerated macho behavior that seems completely illogical...
